I think it means that there is no guaranteed promotion track. Like some people might be on a 5/7/9 or 7/9/11 track, which means that if you're doing okay, you will almost certainly be promoted to the next level after 1 year in the previous one. I hope that helps.

How does one go from an 11 to a 12?  Is it automatic, or must one apply for it?

AA's are on a 9/11/12 but it is all automatic. Don't see many hired at 9, usually the odd straight from school hire, but after a year they go to 11 automatically. GS-11 is fairly common hire point and automatically to GS-12 after a year.

You will see the occasional lightly qualified GS-12 hire with more qualified people brought in at GS-11. Doesn't make sense but it works out.
